What you submit
The form asks for your name, email and state, and lets you optionally provide a phone number, general credit concern, plan of interest and community rate category. It also records your language choice and the time of your request. Please do not include a Social Security number, account number, password, credit report, payment information or identification document.
How we use it
We use your request to contact you about a consultation, understand your interest, and respond in English or Spanish. Submitting the form does not enroll you in a paid service. We do not use this form to obtain consent for marketing texts.
Access and your choices
Requests are stored in the site database and made available to an authorized owner account. To ask about, correct or delete a submitted inquiry, contact 5thavenuecreditco@gmail.com. Do not send sensitive financial records by ordinary email. Any later service would require separate written terms and a secure method for exchanging records.
